Rehabilitation Facilities

There are times when going home after a hospital stay is not safe initially, as more care is needed than can be provided in the home by either family or by a home healthcare provider. Sub-acute rehabilitation facilities and rehabilitation hospitals can prepare patients to return home safely. Your physician and discharge planner from The Valley Hospital will discuss with you and your family if care in a sub-acute rehabilitation facility or rehabilitation hospital is necessary.

Sub-Acute Rehabilitation

Sub-acute rehabilitation programs offer physical, speech and/or occupational therapy. Most of these programs are offered in skilled nursing facilities (SNFs). Sub-acute rehab may also be necessary for those who require long-term IV antibiotics (i.e., 2-4 weeks) but no longer require hospital level of care. Patients who need wound care or who have a change in their medical condition (new ostomy, new insulin-dependent diabetic, or new amputation) may also be appropriate candidates for this level of care.

Rehabilitation Hospitals

Rehabilitation hospitals offer intensive rehabilitative care, such as physical therapy, occupational therapy and speech therapy. The care a rehabilitation hospital provides may be helpful for patients after a stroke, open-heart surgery, amputation of a limb, traumatic brain injury or other severe physical trauma. Patients must be willing and able to participate in three to four hours of vigorous physical and/or occupational therapy each day; have a safe and realistic plan for discharge from the rehabilitation hospital to the community; and have the potential for continued improvement.
