What to Bring to Your Appointment

Please bring these items to your first appointment:

• Medicaid, Medicare, or other health insurance card (if you have one)
• List of medications (or the medications themselves) 
• ID for the patient and for each person in the family (if applying for assistance). These can include: 
  • A valid NJ driver’s license
  • U.S. resident alien card (“green card”)
  • Passport, visa, or US INS document with a date
  • Birth certificate

Before your appointment, we will mail you information and a list of other items to bring if you are applying for financial assistance, charity care, or a health insurance program (Medicaid, Medicare). You will need to bring proof of income, a federal income tax return, bank statements, proof of residency, utility bills, and other items as needed.
