The Care Coordination discharge planning team of case managers and/or social workers will help you arrange for care after your hospital stay by:
Educating you and your family on the options available after your discharge from the hospital.
Your discharge planner will discuss with you services that can be provided upon discharge. Available options and coverage for these services will vary. The most common discharge options include:
- Home healthcare
- Rehabilitation facilities (sub-acute rehabilitation, rehabilitation hospitals)
- Respite care
Conferring with insurance companies.
Our team will help you understand your admission status and insurance coverage.
Making referrals to programs or services that may be of benefit to you.
There are many resources, both within Valley Health System and throughout the community, that may be of benefit to you after your discharge from the hospital. Our team can guide you to the services or programs that may be right for you.
