Primary Care at Home

At Valley, we recognize the need to provide care to our patients wherever possible. With the increasing demand for care to be provided in the home, Valley Medical Group offers Primary Care at Home to meet the needs of the community. 

We provide primary care services to patients at their residences. Our services include the following:

• Routine primary care visits
• Prescription medication management
• Blood work and laboratory testing
• Coordination with imaging companies to provide orders for portable EKGs, X-rays, ultrasounds, and more
• Coordination with other Valley Health System resources to provide home care services like physical and occupational therapy, speech therapy, and wound care management
• Annual wellness visits
• Availability for urgent-care visits and telehealth visits
