Valley Health System congratulates Mary Costa, MS, BSN, RN, NE-BC, Director of Population Health for Valley Health System, on receiving the 2023 Innovations in the Continuum of Care Award presented by the Organization of Nurse Leaders of New Jersey (ONL NJ), and David Rivera, DNP, RN, FNP-BC, Assistant Vice President of Medical/Surgical Services for The Valley Hospital, on receiving the Professional Recognition Award presented by ONL NJ.
According to ONL NJ, the Innovations in the Continuum of Care Award is given to a nurse leader who has implemented a project that has a significant impact outside of the acute care setting. The Professional Recognition Award is given to a nurse leader whose achievements and accomplishments have made a significant impact on the nursing profession.
“Mary has improved care coordination in the transition from acute care to home care and skilled nursing facilities. Her effective nursing leadership has cultivated transparency, broken down silos, and enhanced collaboration and communication among providers across the care continuum for the benefit of our entire community,” said Charles Vannoy, DNP, MBA, RN, FACHE, NEA-BC, HN-BC, Vice President of Patient Care Services and Chief Nursing Officer of The Valley Hospital.
Continued Vannoy: “David is being recognized for his role in several initiatives that have advanced patient care and safety, including innovative AUGi falls-prevention technology at the hospital and the Brain Squad initiative in Valley’s neuro intensive care unit. His leadership, experience, and passion for nursing have resulted in improved patient outcomes, as well as a culture of collaboration and continued learning among Valley’s healthcare teams.”
Costa, Rivera, and other OLN NJ award recipients were recognized on Saturday, December 2, at Forsgate Country Club in Monroe Township, New Jersey
